How to Request Your Mental Health Record

You have the right to request a copy of your mental health records at any time. We strive to keep this process simple, secure, and respectful.

How to request records:
Email your therapist or our administrative team at [email protected] or [email protected] to request a Records Request.

Once received, a team member may review the request with you to explain any risks or benefits, especially if records are being shared with a third party.

Timeline & fees:
Please allow 3-5 business days for your therapist to fulfill any records requests.

  • Electronic Request (PDF Delivery): Flat administrative fee of $35.00 will be charged for records delivered via secure patient portal or encrypted email.
  • Hard Copy: $10.00 for the first 25 pages, $0.50 thereafter
  • Summary of Treatment: If you prefer a written summary of treatment in lieu of full clinical notes, a fee of $40 will be charged to account for the professional time required to prepare the document.
  • Exemptions: Fees may be waived or reduced if the records are required for a Social Security Disability claim or other public assistance programs, provided documentation of the claim is submitted.
